さて、その症例報告というのが以下の英文です。

CASE PRESENTATION

A 50-year-old woman with a past medical history of hypertension, well controlled with diet and exercise, presented with progressive shortness of breath, associated with left-side pleuritic chest pain. On examination, vitals were stable. There was decreased air entry on the left side of the chest; heart sounds were muffled. The laboratory results were unremarkable. The chest radiograph demonstrated a significant opacification on the left side, and mediastinal shift to the right side (Figure 1). The electrocardiogram showed sinus tachycardia with low voltage in precordial leads. The chest CT scan with contrast showed a giant pericardial cyst occupying >50% of the left hemithorax, compressing the lower lobe of the left lung, crossing the mediastinum into the right pericardiac border. The size of the cyst measured 22 x 18 x 15 cm (Figures 2-4). Under video-assisted thoracoscopic surgery, the cyst was aspirated. Approximately 1500 cc of fluid was removed, and then the cyst was dissected away from the left lung, pericardium, and diaphragm, and toward the right lung. The cytology of fluid showed clusters of mesothelial cells with abundant lymphocytes and eosinophils.

試験の結果、活性は安定していた。

胸の左側には空気の侵入が少なくなり、心臓の音が聞こえなくなった。

実験室の結果は特記すべきものではなかった。

胸部X線写真では左側に有意な混濁が認められ、右側には縦隔偏位が認められた(図1)。

心電図では,前胸部に低電圧の洞性頻拍を認めた。

造影CTでは、左胸の50%以上を占める巨大心膜のう胞がみられ、左肺の下葉を圧迫し、縦隔を越えて右心膜境界に交差していた。

嚢胞の大きさは22x18×15cm(図2-4)であった。

ビデオ補助胸腔鏡手術では,嚢胞は吸引された。

約1500ccの体液を除去した後、嚢胞を左肺から、心膜および横隔膜から切り離し、右肺に向かって切除した。

体液の細胞診では,リンパ球および好酸球が豊富な中皮細胞のクラスターが認められた。
